About Coordinated Chronic Disease Prevention
Coordinated Chronic Disease Prevention was established within the National Center for Chronic Disease Prevention and Health Promotion (NCCDPHP) to work collaboratively with state and community partners to efficiently and effectively implement chronic disease prevention and health promotion strategies.
The goals for this coordinated approach are to promote a focused chronic disease prevention and health promotion agenda; maximize efficient use of resources; improve CDC’s support for and share its technical expertise with states and communities; and support chronic disease prevention and health promotion programs in every state to improve population health.
