Contacting ARDI

If you have questions or comments about the Alcohol-Related Disease Impact (ARDI) tool, you can submit your question or comment on the Contact ARDI page.

To send your questions or comments to ARDI:

1. Click the Contact Us about ARDI link at the top right corner of the page. The system displays the Contact Us page.
   Note: The Contact Us about ARDI link is available from any page in ARDI.
   
   
   
   
2. Complete the fields as described in the table below. Required fields are indicated with an asterisk (*).
   
   

   Field	Description
   Name	Enter your name.
   Organization	Select your organization from the drop down list.
   Email address to send replies	Enter your email address if you want a reply to be sent to you. Email address must be in the following format: name@company.com.
   Questions/Comments*	Enter your question or comments.

3. Click Submit. The system submits your question/comment to the ARDI administrator.
