Alternatively, a more conservative value of 10% can be used to reflect the number of people previously screened and aware of their prediabetes status.

Percentage of eligible, screened persons who participate in the intervention:The default value reflects the average participation rate from an employer sponsored demonstration program.

Percentage of eligible, screened persons participating:Among the subgroup of persons who are eligible for the program, this is the percent that will receive program costs and weight loss benefits (which reduce diabetes incidence).

Intervention Weight Loss and Regain ScheduleDefault values reflect average percentage weight loss and regain trends based on data from the National Diabetes Prevention Program.

Enter the cumulative percentage of weight lost each year relative to baseline weight (must be less than 10%):Weight loss is cumulative, relative to the baseline weight (i.e. a value of 0 means that the weight is the same as it would've be without the program). The default values show the greatest weight loss in year 1 with a gradual regain in years 2, 3 and 4. These values are based on persons who participated in the National DPP. Participation is defined as attending at least 4 program sessions. Weight losses greater than 10% will lead to results with low credibility under the current model assumptions.

Risk Group to Participate in ProgramPersons in the following risk groups with a BMI≥24 are eligible to participate.

Persons with prediabetesThe American Diabetes Association and others recommend that persons with prediabetes be referred to a lifestyle change program such as the diabetes prevention program. Persons with prediabetes have a blood sugar level higher than normal, but not high enough for a diagnosis of diabetes (FPG 100-125 mg/dl or A1C 5.7-6.4%). The subset of your population predicted to have prediabetes (and BMI≥24 kg/m²) is determined using a prediction equation and the selected Population Characteristics.

Persons with prediabetes and other persons at risk for type 2 diabetesThis risk group is the largest group as it includes persons with prediabetes and 'other persons at risk for type 2 diabetes.' It is about one and a half times larger than the prediabetes group and has a slightly lower risk of progressing to type 2 diabetes. The CDC's recognized diabetes prevention programs require that at least 50% of participants have prediabetes as measured by a blood test (FPG 100-125 mg/dl or A1C 5.7%-6.4%). The remaining 50% can qualify as 'other persons at risk for type 2 diabetes' based on having a score of nine or higher on the CDC prediabetes screening test.

Persons with high-risk prediabetesThis group is a subset of the group 'persons with prediabetes.' This group is the smallest in size (about 20% of the prediabetes group), but represents those with the highest risk of progressing to diabetes. It is defined by persons with an FPG or A1C on the high end of the prediabetes range (FPG 110-125 mg/dl or A1C 6.0-6.4%).

Program CostsProgram costs do not include screening costs unless the user selects 'Screen persons for prediabetes if they have not been previously screened' in the screening box.

Program cost per person:The default cost of $417 is the cost of a group-based National DPP or similar program.

Average Screening Cost Calculation:The average screening cost is calculated as the sum of the "screening test cost per person" and the "other screening costs per person" multiplied by the "number screened per case detected."

Other screening costs per person:'Other screening costs' include the cost of a brief follow-up visit to discuss the patient's screening test results and receive a referral to a program such as the National DPP. The default cost of $20 only includes the cost of this follow-up visit. The costs of recruitment for the screening program is not included.
